Dakota del Norte, Dakota del Sur, Illinois, Indiana, Iowa, Kansas, Michigan, Minnesota, Missouri, Nebraska, Ohio, Wisconsin

REQUIREMENTS:

1. Valid Passport with at least two (2) blank pages
2. Two (2) copies of the passport, including the information pages and the page with the last Colombian visa or last entrance seal to Colombia.
3. Original and copy of the Visa Application Form signed and completely filled out.

4. Three (3) recent color passport type photographs, white background 3X3 cm (1.2X1.2 inches).
5. Original and copy of the passport data pages with proof of the visa that is desired to be transferred, except for when it is lost, in which case he/she will communicate the information in writing.
6. Two (2) photocopies of the alien identification card (CÉDULA DE EXTRANJERÍA), depending on the case.
7. Certificate of the migratory movement sent by the Administrative Department of Security - DAS issued within three (3) months, except for if the visa to be transferred has not surpassed the term for loss of use by absence of the national territory or not to surpass the term of permanence in the country.

NOTE I: FOR THE CASE OF THE TEMPORARY COLOMBIAN NATIONAL SPOUSE VISA, TEMPORARY REFUGEE VISA, AND RESIDENT VISA HOLDERS THE COMMITMENT TO INFORM THE "DEPARTAMENTO ADMINISTRATIVO DE SEGURIDAD, DAS" ABOUT THE NEW ACTIVITIES TO BE DEVELOPED. TO PRINT FORM CLICK HERE.

NOTE II: IN ALL THE CASES, WHEN THE INTENT IS TO EXERT A REGULATED VOCATION, THE FOREIGNER REQUIRES SPECIFIC AUTHORIZATION IN THE VISA AND FOR IT TO BE EFFECTIVE HE/SHE MUST FULFILL THE SAME REQUIREMENTS STATED IN THE EFFECTIVE NORMS THAT ARE DEMANDED FOR THE COLOMBIAN NATIONALS. THE FOREIGNER WILL PRESENT BEFORE THE OFFICE IN CHARGE OF THE VISA EXPEDITION THE DOCUMENTS THAT ALLOW HIM/HER TO EXERCISE THE REGULATED VOCATION, AS THE AGREEMENT OR CONFIRMATION OF THE PROFESSIONAL TITLE, THE PERMISSION OR PROVISIONAL LICENSE TO EXERT, OR THE MATRICULATION OR PROFESSIONAL CARD.

NOTE III: FOR THE HOLDERS OF OTHER CLASSES AND VISA CATEGORIES, THE FOREIGNERS WILL HAVE TO UPDATE THEIR OCCUPATION WITH THE DOCUMENT THAT CREDITS THE AUTHORIZED ACTIVITY IN THE VISA, AS LONG AS IT CONTINUES WITH THE SAME ORGANIZATION AND OCCUPATION.

NOTE IV: FOR THE TRANSFER OF ADDITIONAL AUTHORIZED ACTIVITY IN THE VISA, DEPENDING ON THE CASE, THE FOREIGN TEMPORARY COLOMBIAN NATIONAL SPOUSE VISA, TEMPORARY REFUGEE VISA, AND RESIDENT VISA HOLDERS WILL HAVE TO FULFILL WITH WHAT IS STATED IN DECREE 4700 OF THE 25TH OF NOVEMBER 2009, CHAPTER VII, NUMERAL 4. THE HOLDERS OF OTHER VISA CLASSES AND CATEGORIES WILL HAVE TO ENCLOSE THE SPECIFIC REQUIREMENTS, ACCORDING TO THE ADDITIONAL ACTIVITY THAT IS ASKED FOR TO AUTHORIZE THE VISA AND ARE STATED IN THE PRESENT DECREE.
