Dakota del Norte, Dakota del Sur, Illinois, Indiana, Iowa, Kansas, Michigan, Minnesota, Missouri, Nebraska, Ohio, Wisconsin

REQUIREMENTS:

1. Valid Passport with at least two (2) blank pages
2. Two (2) copies of the passport, including the information pages and the page with the last Colombian visa or last entrance seal to Colombia.
3. Original and copy of the Visa Application Form signed and completely filled out.

4. Three (3) recent color passport type photographs, white background 3X3 cm (1.2X1.2 inches).
5. Original and copy of letter from the legal representative of the company responsible for the means of international transport indicating the intended activity and its commitment before the Colombian Government to be responsible for the stay of the foreigner in national territory and his/her return to his/her country.
6. Original and copy of the charter certificate and the company`s legal representation.
7. When dealing with a fishing boat, applicant must enclose the additional: original and a copy of the note of the company responsible for the fishing boat, in which it manifests that it fulfills the norms established for the hiring of foreign crew, especially the proportionality certificate. In this case the applicant must enclose the original and a copy of the proof of having informed the maritime authority about the instance.
8. Original and copy of the corresponding permissions for the implementation of the activity.

NOTE I: THE CREWMEMBER OF AN EMBARKATION WITH THE COLOMBIAN FLAG ADMINISTRATED BY A NATIONAL OR FOREIGN SHIP OWNER, SHOULD ADDITIONALLY ENCLOSE A NOTE BY THE SHIP OWNER OR THE COMPANY RESPONSIBLE THAT INDICATES THE ENROLLMENT CONTRACT TERM, SPECIFICALLY THE JOB DESCRIPTION AND THE COMPETENCE OF THE CREWMEMBER.
