Atendemos 12 Estados:

Dakota del Norte, Dakota del Sur, Illinois, Indiana, Iowa, Kansas, Michigan, Minnesota, Missouri, Nebraska, Ohio, Wisconsin

REQUIREMENTS:

1. Valid Passport with at least two (2) blank pages
2. Two (2) copies of the passport, including the information pages as well as the page with the last Colombian visa or last entrance seal to Colombia.
3. Original and copy of the Visa Application Form signed and completely filled out.

4. Three (3) recent color passport type photographs, white background 3X3 cm (1.2X1.2 inches).
5. Original and copy of the Work Contract, or Administrative Act (Acto Administrativo) or the form "Contract Abstract," depending on the case, signed and notarized.
6. Original and copy of the company`s legal existence certificate and legal representation, or a valid document that credits the legal function of the company, depending on the case, when the contractor is a legal entity, it must be issued within the last three (3) months.
7. When the contractor is an individual he/she must prove that his/her monthly income is above to ten (10) valid legal minimum wages.
8. Original and copy of the Colombian Ministry of Social Protection certification, indicating the company`s current proportionality between national and foreign workers is respected, in agreement with what was established in the Worker`s "Compensation Code" Código Sustantivo de Trabajo, except for when it deals with cultural groups, public, artistic, or athletic spectacles. This request exonerates this requirement when the hiring organization is an appendage, entity, or institution of the State.
9. Two (2) photocopies of the provisional permit or license to implement vocation or corresponding activity in Colombia, granted by the competent authority, depending on the case, when it is related to occupations or regulated trades, or the professional title, if the occupation is non-regulated or the suitability certificate is issued by the hiring organization or employer based on the applicants experience.
