Atendemos 12 Estados:

Dakota del Norte, Dakota del Sur, Illinois, Indiana, Iowa, Kansas, Michigan, Minnesota, Missouri, Nebraska, Ohio, Wisconsin

REQUIREMENTS:

1. Valid Passport with at least two (2) blank pages
2. Two (2) copies of the passport, including the information pages and the page with the last Colombian visa or last entrance seal to Colombia.
3. Original and copy of the Visa Application Form signed and completely filled out.

4. Three (3) recent color passport type photographs, white background 3X3 cm (1.2X1.2 inches).
5. Two (2) photocopies of the "CONSTANCIA DE ASIGNACIÓN" or Certificate of Assignment of the minor to the family. This is sent by the adoption agency in Colombia or by the INSTITUTO COLOMBIANO DE BIENESTAR FAMILIAR, ICBF.
6. Two (2) photocopies of the proof of legal existence of the Adoption Agency in Colombia "CERTIFICADO DE PERSONERIA JURIDICA". If it is through the ICBF (INSTITUTO COLOMBIANO DE BIENESTAR FAMILIAR) this requirement is not need.
7. Original and copy of the letter from the adopting parents stating the purpose of the trip.
8. Two (2) photocopies of the return ticket to the country of origin.

NOTE: DOCUMENTS THAT ARE NOT OF COLOMBIAN ORIGIN MUST BE CERTIFIED WITH AN APOSTILLE OR ATTESTATION OF AUTHENTICITY OF THE DOCUMENT. FOR APOSTILLE INFORMATION, CLICK HERE
