Atendemos 12 Estados:

Dakota del Norte, Dakota del Sur, Illinois, Indiana, Iowa, Kansas, Michigan, Minnesota, Missouri, Nebraska, Ohio, Wisconsin

REQUIREMENTES:

1. Valid Passport with at least two (2) blank pages
2. Two (2) copies of the passport, including the information pages and the page with the last Colombian visa or last entrance seal to Colombia.
3. Original and copy of the Visa Application Form signed and completely filled out.

4. Three (3) recent color passport type photographs, white background 3X3 cm (1.2X1.2 inches).
5. Original and copy of the Civil Record or document that demonstrates the kinship or link (i.e. marriage certificate, birth certificate). It must be notarized and certified with an APOSTILLE from the Secretary of State of the state where it was issued.
6. Proof of economic dependency (i.e. copy of the tax form statement).
7. Original and copy of the letter from the holder of a Colombian visa (petitioner) to confirm his/her permanent responsibility of the beneficiary and guaranties his/her/their departure of the country.
8. Photocopy of the Colombian visa granted to the petitioner.
